Who we are
Larkspur Mobility LLC, Ashford Hills, OH, provides non-emergency medical transportation. This policy explains what we collect when you use this website or book a ride.
What we collect
- Booking details — name, phone, email, pickup and destination addresses, appointment times, mobility needs and any notes you add.
- Payment details — processed by our booking provider; we do not store full card numbers on this website.
- Website data — standard server logs and, where enabled, analytics. No third-party analytics or advertising trackers are installed on this site.
How we use it
To quote, schedule and run your trip; to contact you about that trip; to bill you, your insurer or your care coordinator; and to meet our record-keeping obligations.
Who we share it with
Payment processors, and your insurer or care coordinator where you have asked us to bill them. We do not sell your information or share it for marketing purposes.
Health information
Transportation bookings can reveal health information. Where we bill Medicaid or a Medicaid-managed-care plan directly, Larkspur Mobility acts as a HIPAA Business Associate and maintains a Business Associate Agreement with that payer.
How long we keep it
We keep trip and billing records for seven years to meet insurer and Medicaid record-keeping requirements, and delete marketing contact details on request.
